Summer 2016 Quality Improvement Workshop – Jackson

 

The Summer 2016 Quality Improvement Workshop is a core part of our program and essential to everyone's success. During registration you will choose one of two "tracks" of content to participate in at your workshop. Please find details on "Track One" and "Track Two" below.

 

You may register to participate at a cost of $500 per attendee. We recommend including your ACO Champion, Care Coordinator, and Practice Manager. Please take the time to register before June 30th. 

 

IMPORTANT: Remember to bring at least one laptop for your site or facility to your Quality Improvement Workshop.

 
 Start Time (Track I): 9:00AM

Start Time (Track II): 10:00AM

In order to provide better coverage of the training materials, Track I participants will arrive an hour early, after which each track will run simultaneously until the workshop ends at 4:00PM. Coffee and tea service will be available in the Track II workshop space where participants travelling with their Track I colleagues may arrive early and await the start of their track.

Track One: Clinical Health Coach Training (CHCTO) for National Rural ACC and Caravan Health with a Brief Overview of CCM and TCM Billing

Prerequisite: Must be in the Care Coordinator role and have completed through Module IV in the ICCC training to qualify for Track 1

Designated care coordinators enrolled in CHCTO are required to participate in a One-Day Live Coaching Improvement Workshop to be held in conjunction with the Summer Quality Improvement Workshop in their region. This added learning experience will provide further coaching skill development and practice opportunities as well as assist enrollees to prepare for the Competency Evaluation.

Track Two: Annual Wellness Visit Workshop with Chronic Care Management and Transitional Care Management Billing

 

At this workshop, you will learn about services you can provide to your patients to promote health and wellness. By providing these services, you will improve the health of your community, increase practice revenue and improve your quality scores. You will develop an individual work plan to incorporate these services in your primary care practice.
