Annual Wellness Visit

 

The Spring 2017 Quality Improvement Workshops are now open for registration!

At this workshop, you will learn about services you can provide to your patients to promote health and wellness. By providing these services, you will improve the health of your community, increase practice revenue, and improve your quality scores. You will develop an individual work plan to incorporate these services in your primary care practice.

  • Annual Wellness Visits for Medicare Patients
  • Advanced Care Planning
  • PHQ-9 Depression Screening
  • Fall Risk Screening
  • Smoking Cessation Counseling
  • Obesity Counseling
  • Screening for Substance Abuse and Behavioral Health
  • RAF Scoring
  • HCC Coding

You will also learn about updated guidelines and quality measures for the upcoming year, and have the opportunity to network and learn from fellow Caravaneers regarding best practices for Annual Wellness Visits. By providing these services, you will promote effective management of your community population while increasing practice revenue which will assist with sustaining your Community Care Coordination efforts.

 All attendees will participate in a single track for the full day. Learning objectives on the roster are as follows:

 

Annual Wellness Visits 101

Understand how Annual Wellness Visits will improve your patient outcomes, quality scores and generate important revenue for your health system. Learn how to address 50% of your quality scores in a single visit. Learn how to ensure 50% of your patients have Annual Wellness Visits. Learn about what other services can be performed as part of an Annual Wellness Visit, and how to generate practice income to pay for the staff needed to perform the service and manage population health.

 

Annual Wellness Visits 102

 

Address updated guidelines and opportunities for 2017, RAF scoring, HCC Coding, and performing SBIRT in a primary care setting.

 

Quality Measures and Expectations for 2017

 

Review 2017 Quality Measures for your specific program and discuss anticipated challenges and tips for success.

 

Annual Wellness Visit Workflows and Barriers Workshop

 

Share Best Practices and potential barriers to overcome. Understand the barriers to implementing your Annual Wellness Visit program. Develop an individual work plan for recruiting patients, incorporating the Annual Wellness Visit in acute provider visits and maximizing revenue and quality performance.

 

Lightbeam 101

Practice building a Care Plan in the Care Management Module. Search for ACO Quality Measure Gaps and potential patients for CCM in Financial Analytics.

 

Advanced Payment Models

Discussion and development from moving volume to value.

WORKSHOP CANCELLATIONS

Caravan Health reserves the right to cancel a workshop/class when the required minimum enrollment is not met.  Earliest notification will be given and the participant will receive a full workshop ticket refund when applicable.

 

 


If a facilitator needs to cancel a workshop/class or there is an inclement weather delay or closing, attendees registered in the workshop/class will receive an email at the address they listed as their preferred email address on the registration form.  We will give as much notice as we can about cancellations.
