Part 2: Complete your paperwork

Join an end-of-life expert from Hospice of the Northwest for a two-part workshop in Advance Care Planning. 

Advance care planning ensures your goals, values and preferences are known, not only at the end of life but if you become unable to communicate those desires due to illness or injury. Join us for part two - workshop to learn more about advance directives and walk away with your completed forms.
