Join us for an informal meet-up surrounding how this pandemic has affected healthcare social workers. Tracy Grafton, LCSW, ACHP-SW, and Stephanie DePiano, LCSW, ACHP-SW, will facilitate a conversation on how to continue to provide the best and most useful services, and how to support each other during this unprecedented time in healthcare social work.

Stephanie DePiano is the Social Work Manager for Hackensack Meridian Health Palliative Care services. Stephanie graduated from Rutgers University in 2003 with her Masters in Social Work. Stephanie is a Licensed Clinical Social Worker and holds the new Advanced Palliative Hospice Social Work certification. She began her medical social work career at Somerset Medical Center where her passion for working with those coping with serious illness was ignited. In 2007 she joined the Meridian Health family. She has held several positions within Meridian, starting at Ocean Medical Center and then becoming Director of Social Services at Meridian Nursing and Rehab at Brick. In 2012 she joined the Meridian Care Journey program as the first social worker with the CMS Palliative Care demonstration project which has brought Palliative Care into the home for more than 3500 patients. Stephanie has led numerous projects recognizing National Health Care Decision Day, National Hospice and Palliative Care month, and Compassion Fatigue. In addition to supervising the social workers on her team, she believes strongly in educating and empowering other social workers in the field through clinical supervision and speaking engagements. She has previously presented at a national level at the Social Work Hospice and Palliative Care Network (SWHPN) General Assembly as well as The American Academy of Hospice and Palliative Medicine (AAHPM).
Tracy Grafton, LCSW, ACHP-SW, is a social worker who has worked for Hackensack Meridian Health recently transferring from Haven Hospice/JFK Medical Center to the Palliative Care Program at Ocean Medical Center. Since earning her MSW from Rutgers, The State University of New Jersey, Tracy worked in hospice for the past 20 years in several roles, including social work, oncology support, child/adolescent and adult bereavement, bioethics member and education. As an Adjunct Faculty member at both Rutgers University and The College of New Jersey, she has taught on Aging, Oncology, End-of-Life, & Bereavement for all ages. Tracy is a community educator who is active with the Institute for Healthcare Improvement’s “The Conversation Project”, the New Jersey Health Care Quality Institute’s “Conversation of Your Life” task force, and all efforts promoting its importance. A native of Youngstown, Ohio, Tracy is married to her college sweetheart, William Grafton, has two children, under age 30, who have their advance directives. She is an avid cyclist who enjoys long-distance biking, travel, reading and music.
