Abstract: Many simulation programs have recently shifted towards providing remote simulations with virtual debriefings. Virtual debriefings require simulation educators to facilitate discussions through web-based videoconferencing platforms. Facilitating debriefings through a computer interface introduces a unique set of challenges. Simulation educators require practical guidance to support meaningful virtual learning in the transition from in-person to virtual debriefings. Abstract: Simulation is a tool that is often used for healthcare training and education. In addition to the clinical application, simulation can serve as a vehicle for the evaluation of system-based processes. System-focused simulation, also referred to as process-oriented simulation, can be accomplished through Simulation-Based Clinical Systems Testing (SbCST), and Simulation-based Hospital Design Testing (SbHDT).

System-focused simulations use systems engineering, human factors, and risk management principles to explore and achieve excellent clinical care, enhanced patient safety, and improved outcome metrics across a healthcare system. SbCST is conducted in situ to evaluate clinical system and care processes prior to occupancy to mitigate risks whereas SbHDT simulations are conducted in the pre-construction phase of design development to evaluate the built environment to improve safety and optimize efficiency. 

There are many benefits to system-focused simulation such as preventing harm, increasing effectiveness, improving efficiency, supporting safety, streamlining processes, decreasing errors, and controlling costs. In order to integrate systems-focused simulations into a program, it is necessary to overcome barriers and challenges: support from senior executives, buy-in of stakeholders, avoiding a disruption to patient care during in-situ sessions, availability of equipment, and the intensity of resources. 

This presentation will discuss how and why to use system-focused simulations, explore the barriers and facilitators impacting teams interested in this format, and describe a reporting structure used by a Simulation Center that has achieved accreditation in Systems Integration by the Society of Simulation in Healthcare (SSH). In addition, participants will be shown examples of systems-focused simulations and their findings and outcomes, as well as the impact on patients and healthcare providers.

- Define the intent of systems-focused simulations including Simulation-Based Clinical Systems Testing and Simulation-Based Hospital Design Testing with particular reference to their role in improving clinical practice and patient safety.

Abstract: Health inequity has been well documented as a public health issue however it has recently garnered increased attention due to the health disparities brought to light during the COVID-19 pandemic. An important strategy for addressing health disparities and bias?is to start with educating our current and future healthcare providers.?Although health equity is a common theme that institutions strive to achieve, many educators have difficulty finding meaningful teaching strategies and activities to address the subject. Through the use of simulation, learners can practice a variety of scenarios such as providing care to diverse patients to managing situations where learners witness racism in the clinical setting.? Teaching strategies including creating simulation experiences that can promote cultural humility are included in the presentation. Incorporating themes such as low socioeconomic status, LGTBQ identity issues will allow learners to reflect on their own bias, lived experiences, and negative stereotypes. This presentation will discuss how to create an inclusive learning environment, provide examples of simulation activities that can promote cultural humility, and describe strategies for mitigating bias.

Abstract: This presentation will provide an overview of the vision for a proposed NPSB and highlight how this agency would advance healthcare simulation technology in support of improving patient safety and reducing medical errors at a national scale.

The COVID-19 pandemic and its related public health crisis has demonstrated a pressing need to improve safety in our health system, especially in the ways it has exacerbated an already-high medical error rate, with an estimated 250,000 deaths a year in the U.S. due to preventable medical error prior to the pandemic. The lack of a coordinated and systemic answer to improving safety in health care points to the need for the U.S. federal government to establish an apolitical, independent, and centralized capability to protect patients and healthcare workers–one that delivers rapid solutions to safety threats and spurs adoption of new innovations in technology. Looking to the gains that the transportation industry has made with their safety record, due in part to the efforts of their National Transportation Safety Board, we believe that a similar model could be applied to the healthcare industry. This proposed independent federal agency, the National Patient Safety Board (NPSB), would support agencies in monitoring and anticipating adverse events using artificial intelligence, conduct studies, create recommendations and solutions to prevent medical error, and leverage existing systems to bring key learnings into practice.

A critical element to the future of patient safety lies in the vast potential to tap into new and existing technologies. The gains made over the past two decades in health information technology and analytical tools provide us with new pathways to markedly support healthcare workers with a modernized 21st-century approach. This agency would focus on exploring and accelerating the adoption and implementation of existing and new technologies that would lead to significant improvements in patient safety across all healthcare settings. Leveraging and proliferating the use of healthcare simulation, which ensures high-quality education and training for healthcare professionals to master cognitive, technical, and behavioral skill sets through technologically advanced and crafted experiences, can be an effective method to improve patient safety. This model of experiential learning provides precise and intentional learning environments to better equip the healthcare workforce of the future with the confidence and skills to practice to the best of their ability.
