Summer 2017 Quality Improvement Workshops are now open for registration!
Join us to learn about services you can provide to your patients to promote health and wellness. Patients with behavioral health issues are challenging to treat and consume a disproportionate share of resources. There is a tremendous opportunity to improve their care by embracing behavioral health integration. We’ll be discussing how to take advantage of new billing codes to improve behavioral health services, improving patient satisfaction, and advancing care information.

All attendees will participate in a single track for the full day. Learning objectives on the roster are as follows:
Collaborative Care Management and Behavioral Health Integration
Understand what Behavioral Health Integration means, how it can help your patients, and how to bill for it. Develop a plan specific to your practice with the PDSA.
Patient Satisfaction
Master the principles of patient satisfaction. Review your current scores and discuss solutions with peers. Learn what drives satisfaction in your practice and how you can affect it.
Advancing Care Information
Understand the new requirements around Advancing Care Information and how they apply to your practice. Understand the importance of electronically connecting with other practices and hospitals. Learn how to apply principles to your practices.
Roundtable Discussion: Advancing Care Information Through Effective Transitions of Care
Share Best Practices and potential barriers to overcome. Work with others to improve your processes around TCM. Share success stories and brainstorm solutions for what works in your area.
MIPS Checkpoint
Review your progress to date on quality measures. Identify steps to take to improve your reporting process. Discuss successful QI strategies with other practices.
Registration and Travel Reimbursement Guidelines
AIM/ACO Clients must register at least 1 attendee to participate.
Starting January 1, 2017, CMS updated their requirements for the reporting of ACO related expenses and eligible reimbursements. Travel expense for ACO related activities is an eligible expense to be reimbursed through the updated ACO Reimbursement policy (this will count toward your community’s quarterly eligible reimbursement maximum). Please follow your individual community’s internal guidelines for specific travel accommodation rules and questions.
For your reference, please find the Caravan Health Excel template that will serve as a cover page for all reimbursement submissions HERE. To submit for reimbursement following your travel, please send the completed Caravan Health Excel template and all supporting documents to ACOreimbursements@caravanhealth.com . Please note, all travel expenses must be submitted no later than 10 days following the end of the calendar quarter in which they were paid. [Example: Travel to the April 2017 Quality Improvement Workshop must be submitted for reimbursement no later than July 10, 2017.]

PTN/TCPI Clients may register up to 3 total attendees per clinic to participate.
The participant is eligible for reasonable reimbursement for travel expenses associated with TCPI-required travel to quarterly Quality Improvement Workshops.
Reasonable travel reimbursements will include one (1) car rental, meals, mileage (if using personal vehicle), and the U.S. General Services Administration (GSA) per diem for up to three (3) attendees total regardless of number of clinics. If your facility is over 250 miles from your Quality Improvement workshop, airfare and hotel accommodation for one night may be optionally submitted for reimbursement.
All reimbursement must be submitted using NRACC's Standard Reimbursement Template (download HERE) within 30 days of costs incurred. For your reference, please find the GSA Per Diem Guide on our Pre-Launch Resources page. To submit for reimbursement following your travel, you will fill out and send our Standard Reimbursement Template to AccountsPayable@CaravanHealth.com. CPC+ Clients may register up to 3 total attendees per clinic to participate.
CPC+ clients are not eligible at this time for travel reimbursement to the Quality Improvement Workshops. If you have any questions or need clarification, please contact QIWorkshops@CaravanHealth.com.
IMPORTANT: Remember to bring at least one laptop for your site or facility to your Quality Improvement Workshop.

WORKSHOP CANCELLATIONS
Caravan Health reserves the right to cancel a workshop/class when the required minimum enrollment is not met. Earliest notification will be given and the participant will receive a full workshop ticket refund when applicable.
If a facilitator needs to cancel a workshop/class or there is an inclement weather delay or closing, attendees registered in the workshop/class will receive an email at the address they listed as their preferred email address on the registration form. We will give as much notice as we can about cancellations.
