Value Based Payment Program Readiness
 

MACRA, Physician Contracting, and HCC Coding 

 

The Fall 2016 Quality Improvement Workshops are now open for registration!

At this workshop, you will learn about the upcoming payment reform initiatives that will impact your physician practices and future health care system reimbursement. Through facilitated dialogue, you will gain understanding of how to revise your current physician contracting system to better align with the new value based payment systems in a way that will be accepted by the clinicians. Your team will learn how the Medicare Risk Adjustment System works and how to get paid correctly through effective recording of patient problem lists and coding of diagnoses. Staff will learn how to identify high-value post-acute care facilities and encourage patients to seek the best post-acute care providers.

Track Information

 All attendees will participate in an overview of MACRA and HCC coding at the beginning of the workshop, after which they will be split into separate tracks according to their registration.

 

Overview of MACRA and HCC Coding

All participants suggested to participate

Understand how MACRA impacts physician practices and future revenue for your health system. Learn the basics of Medicare risk adjustment and how HCC codes are assigned to patients. Learn about how HCC coding affects your risk scoring and subsequent impact on value based reimbursement models. Understand how to accurately assign HCC codes through improved documentation.  

 

Track I: Physician Contracting Under Value Based Payment

CEO’s, Physicians, and ACO Champions suggested to participate

Participate in a discussion with Horty-Springer attorneys discussing value based payment systems and effective alignment with physician contracting. Develop physician contracting strategies, discuss timelines for implementation, and report innovative ideas to larger group.

 

Track II: Post-Acute Care

Practice Managers and Care Coordinators suggested to participate

Learn about post-acute care opportunities and issues, learn about relative value of PAC setting and develop strategies for improving PAC utilization. Develop Post-Acute Care strategies for individual communities, discuss timelines for implementation, and report innovative ideas to larger group. 

 



Registration and Travel Reimbursement Guidelines

 

ACO Clients may register up to 6 total attendees per clinic to participate.

 

Travel reimbursements will include 1 car rental, lunch, mileage (if using personal vehicle), and GSA per diem for up to 6 attendees total. If your facility is over 250 miles from your Quality Improvement Workshop, airfare and hotel accommodation for one night are optional for reimbursement. For your reference, please find the GSA Per Diem Guide on our Pre-Launch Resources page. To submit for reimbursement following your travel, you will fill out and send our Standard Template Invoice to AccountsPayable@CaravanHealth.com.

 

PTN Clients may register up to 5 total attendees per clinic to participate.
