The Fall 2016 Quality Improvement Workshops are now open for registration!

At this workshop, you will learn about the upcoming payment reform initiatives that will impact your physician practices and future health care system reimbursement. Your team will learn how the Medicare risk adjustment system works and how to get paid correctly through effective recording of patient problem lists and coding of diagnoses. You will learn about services you can provide to your patients to promote health and wellness. By providing these services, you will improve the health of your community, increase practice revenue and improve your quality scores. You will develop an individual work plan to incorporate these services in your primary care practice. You will also learn about the benefit of providing Transitional Care Management and Chronic Care Management services for your patients. By providing these services, you will promote effective management of your community population while increasing practice revenue which will assist with sustaining your Community Care Coordination efforts.

·         Understand how Annual Wellness Visits will improve your patient outcomes, quality scores and generate important revenue for your health system.

·         Learn how to address 50% of your quality scores in a single visit.

·         Learn how to ensure 50% of your patients have Annual Wellness Visits.

·         Learn about what other services can be performed as part of an Annual Wellness Visit, and how to generate practice income to pay for the staff needed to perform the service and manage population health.

 

Annual Wellness Visit Workflows and Barriers Workshop

·         Understand the barriers to implementing your Annual Wellness Visit program.

·         Develop an individual work plan for recruiting patients, incorporating the Annual Wellness Visit in acute provider visits and maximizing revenue and quality performance.

Overview of Transitional Care Management (TCM) and Chronic Care Management (CCM) Billing

·         Understand how TCM can help reduce unnecessary readmissions and promote CCM referrals while generating a higher level of practice revenue

·         Learn how CCM can help improve the care of patients with 2+ chronic diseases and provide added revenue to fund your care coordination program efforts

·         Learn about the detailed requirements specific to Fee for Service practices, Rural Health Clinics and Federally Qualified Health Centers that will assist you in designing your own individual work plan for implementing these services

 

Overview of Risk Adjustment (All participants)

4:00 PM – 5:00 PM

·         Learn the basics of Medicare risk adjustment and how HCC codes are assigned to patients.

·         Learn about how HCC coding affects your risk scoring and subsequent impact on value based reimbursement models

·         Understand how to accurately assign HCC codes through improved documentation
