Congratulations on your nomination as a Provincial Development or Canadian Development athlete and eligibility for services from the Canadian Sport Institute!
Each newly nominated athlete must participate in the Athlete Intake before they are able to access Canadian Sport Institute benefits, programs, and support services.
This will be a 1 hour interactive presentation on what Canadian Sport Institute can offer to help athletes get on National Teams and Power Podium Performances.
Step 1:
- Sign up for a Webinar time that meets your needs from the list above and input all details into the online form.
Step 2:

Please print off and sign the Athlete Agreement Form found attached to your registration email and send (scan or send photo) to vancouverathletes@csipacific.ca for Vancouver athletes and whistlerathletes@csipacific.ca for Whistler based athletes, BEFORE the intake. Athletes under the age of 19 MUST have a parent/guardian sign the form or they will NOT receive an athlete card.
Step 3:
- Please email a headshot/picture of yourself that can be used for the athlete card to vancouverathletes@csipacific.ca for Vancouver based athletes andwhistlerathletes@csipacific.ca for Whistler based athletes.
Step 4:
- Sign in to the webinar online at least 5 minutes early for your scheduled intake time so that you are not holding up the group and the intake can finish on time. *Note: you will be sent the webinar link to the email address provided prior to the intake.
After completeing all four steps your Canadian Sport Institute athlete card will be mailed to your home address (the address you put in this form).

- North Vancouver
- Port Coquitlam
- Port Moody
- Richmond
- Vancouver
- West Vancouver
