Purpose: At this workshop, you will learn about services you can provide to your patients to promote health and wellness. By providing these services, you will improve the health of your community, increase practice revenue and improve your quality scores. You will develop an individual work plan to incorporate these services in your primary care practice.

Annual Wellness Visits 101
10:30 AM – 12:00 PM
· Understand how Annual Wellness Visits will improve your patient outcomes, quality scores and generate important revenue for your health system.
· Learn how to address 50% of your quality scores in a single visit.
· Learn how to ensure 50% of your patients have Annual Wellness Visits.
· Learn about what other services can be performed as part of an Annual Wellness Visit, and how to generate practice income to pay for the staff needed to perform the service and manage population health.
