Those involved in running small, rural, and independent practices who want to learn how to take advantage of new payment models, like CPC+, PTN, TCPI, or ACO’s. This includes physicians, medical directors, practice managers, care coordinators, quality managers.
Learn How to Succeed Under Value-Based Payments
The Caravan Health Accountable Care Symposium is the premier learning and networking event for current and future participants in Advanced Payment Models, particularly from independent practice associations, small practices, rural health systems, independent community hospitals and multi-specialty groups. The Symposium brings together hundreds of ACO participants including primary care physicians, community hospital CEOs, nurse care coordinators and executive leaders to reflect on their successes and challenges. They will be joined by participants in Comprehensive Primary Care Plus, representatives from the Center for Medicare and Medicaid Services, and physicians and hospital leaders that are preparing to enter into Advanced Payment Models in 2018.
At the end of day one, and continuing on day two, the program will showcase programs that will continue to drive better care at lower costs and generate important revenue, including behavioral health integration using the new psychiatric collaborative care models. These programs will be presented in workshop format, with developmental activities to complete during the workshop and upon returning to the practice. These workshops are intended to assist the practice in implementing and billing for these important new primary care services. Further assistance will be available to participants through either Caravan Health’s ACO and CPC+ programs or, if the practice is not participating in an Advanced Payment Model, at no cost through Practice Transformation Networks.
Onsite registration will be open on Tuesday, January 10th from 6 – 8 PM and continue through Wednesday, January 11th beginning at 7:00AM in Highland 1.
