GKAS 2018 Dental Office Registration

In 2018, Give Kids A Smile day will take place on Friday, February 2nd, 2018, at participating locations statewide. Deadline to register is November 6th, 2017.

Please note that both state and ADA registration is required for entry into the supply lottery. By completing this registration form, your office will be registered with the state and be listed on NJDA marketing materials. After completing this form please visit the link below and click on "Sign up" or "Enter event data" to complete registation with the ADA.

When registering for Give Kids A Smile, please note that:

• Your "office contact number" is the phone number used for NJDA to reach your GKAS contact.
• Your "patient call-in number is the phone number used for patients to schedule an appointment.
