CCHC invites you to help us kick-off our Healthier Together Community Forum Series! We will address how the safety-net is responding to the health disparities in our communities and ways we can continue to provide more culturally responsive care. Join us for a conversation about community health workers (CHWs) on Thursday, November 16!

Together, we will explore how CHWs are being used to improve care for our communities. How can we best integrate CHWs and primary care? What are our biggest challenges and obstacles? How can organizations, safety-net clinics, nonprofits, and patients work together to maximize the impact of CHWs for communities of color and other under-served populations? What can YOU do to help?

The Coalition of Community Health Clinics (CCHC) is a network of 15 community clinics working together to improve healthcare for vulnerable populations residing in the Portland region. We achieve this mission by strengthening community health centers, promoting health equity, and fostering collaboration across the safety-net delivery system. Our clinics provide high quality, culturally appropriate healthcare to low-income children and adults who are uninsured, on Medicaid, or under-insured, and to populations struggling with poverty, language barriers, abuse, addictions, homelessness, or mental health issues. Coalition clinics offer primary, preventive, and acute medical services from a broad range of health traditions: allopathic (conventional) medicine, naturopathic, chiropractic, acupuncture and Chinese medicine.
